As part of our service guarantee, you may return, exchange, or replace any
product bought
from us for any reason within 30 days! NOTE: This is NOT a
warranty form!

- Items must be returned in their ORIGINAL CONDITION AND PACKAGING within 30 days of shipment
receipt. Shipment costs are non-recoverable & non-refundable.

and include this form with your return. Please include a copy of your packing slip as
well.
- If your order was paid for with a credit card, the same card will be credited, less card processing fees.
- For your protection, we recommend that you send your returns by UPS or Insured
Parcel Post
- Allow 10-14 business days for your return to be processed.
- If you have received an item that was damaged during transit please contact
UPS or POST OFFICE to make a claim, then contact us for a replacement item.
